Sponsored Account Request Form Details

The UCF Account/Access/Termination Request Form’s purpose is to provision, or de-provision an account (user name) to associates, consultants, contractors, or guests of the University of Central Florida.  The process notifies the sponsor and user that an account was created or disabled, but does not provision any services to the account.  In other words, each service must be separately requested after the account is created.

After the sponsor and the user fill in the UCF Account/Access/Termination Request form and receive the newly created account (user name), the sponsor can begin requesting access for the user by following the below steps for each item that you marked:

Security Authorizers

Resource Authorization Guide

PeopleSoft Access

(Complete all sections. FERPA course SR100W and a basic navigation course are required before working with institutional data (academic records.))

Requester/Sponsor: After the system generates an EmplID, the requester will need to contact their PeopleSoft functional lead or Departmental Security Coordinator (DSC) contact within the department to provision the needed access. Information on DSC’s can be found here.

Individual needing access: Contact srtrain@ucf.edu to enroll in the required SR100W (FERPA Training) and basic navigation courses.  More information is found here: http://registrar.ucf.edu/student-records-training

Access to Institutional Data (e.g., academic records)

(Complete all sections. NOTE: FERPA course SR100W is required before working with institutional data (academic records.))

Access to Human Resources Data

(Complete all sections with HR’s Security Authorizer / DSC approval)

Requester/Sponsor: After the system generates an EmplID, the requester will need to contact their PeopleSoft functional lead or Departmental Security Coordinator (DSC) contact within the department to request the appropriate HR access. Information on DSC’s can be found here.

Individual needing access: No action needed.

College/Departmental Server

(Complete all sections)

Requester/Sponsor: Once the sponsor receives the user account creation notification they must contact the system owner or Security Authorizer (SA) to request the appropriate access to resources (file shares, servers, etc.) or federated applications (Webcourses, etc.) Information on DSC’s can be found here.

Individual needing access: No action needed.

VPN (Virtual Private Network) for remote access

(Complete all sections)

Requester/Sponsor: No action needed after form is submitted.

Individual needing account: Once the user receives their NID, he or she will need to reset their password by visiting http://mynid.ucf.edu  before gaining access to VPN. Once their NID password is set, follow the instructions in KB0010298 to configure the VPN connection.

Wireless access on campus

(Complete sections 1, 3, & 6)

Requester/Sponsor: No action needed after form is submitted.

Individual needing access: Once the user receives their NID, he or she will need to reset their password by visiting http://mynid.ucf.edu  before gaining access to the UCF_WPA2 wireless network.

Enterprise email account

(Complete sections 1, 3, & 6)

Requester/Sponsor: Ask your department’s ePAF originator to submit the “Enterprise Email – Add User Account eForm” to create an email account for the new user.

Individual needing an email account: Once the user receives their NID, he or she will need to reset their password by visiting http://mynid.ucf.edu  before gaining access to the UCF enterprise email account. The user will not be able to access their email account until the ePAF originator receives a notification indicating the email request is complete. Contact your ePAF originator to check on the status of your email account request (consult your supervisor if you are unsure of who this is.)
